Las Vegas has 10 authorized passport acceptance facilities. First-time applicants file Form DS-11 in person — no mail submissions accepted. Bring proof of U.S. citizenship (a certified birth certificate or naturalization certificate), a valid government-issued photo ID, a 2×2 passport photo, and the $165 application fee: $130 by check or money order payable to the U.S. Department of State, plus a $35 execution fee paid directly to the facility.
